Helping to Connect People and Service Providers
Southwest Michigan Long Term Care Connection (SWMLTCC) is a central source of information and guidance for older adults and persons with a physical disability, as well as families, professionals, and chosen advocates. We are able to be the single phone you make, connecting you to every kind of service, answering questions about care, providers or funding sources, and assisting you with accessing the services and financial assistance for which you may be eligible.
Our trained Information Specialists can locate information on topics such as caring for people with memory loss, what Medicare covers, and how a Power of Attorney works. In addition, we are the source for determining eligibility for the MiChoice Program (also known as the Home and Community-Based Waiver). And we can assist you in looking at all sources of public funding available to you as an older adult or person with a disability, to ensure you receive the best quality of care.
We know the importance of planning for long term care, and will help you to obtain care and services in the setting, and with the providers, with which you are most comfortable. This may occur within a short time period - for example, helping you to understand housing and care options for someone leaving a hospital or nursing facility. Or it may happen within a longer time frame, as you plan for future long term care needs. Regardless of your situation, we can help you choose and control the support you prefer in your later life.

- Following up with you to assure that you are happy with the service you receive, check that you are faring well, and provide help in adjusting your plan when you wish to

Our aim is to enable you to make an informed choice by:
- Listening to your unique situation, needs and preferences
- Assessing your needs in a variety of areas, such as daily living, housing, etc.
- Explaining all options for care settings, housing, supportive service providers, and financing
- Focusing on the needs and preferences of the person receiving care
- Advocating for your choice of services, and helping you get around barriers to finding and accessing services
- Providing information that helps you to plan for the future

- We can help prepare the information and documents you will need for publicly funded programs, including those funded by Medicare and Medicaid
- We can help you transition from one setting to another that best meets your needs
