CARE MANAGEMENT PROGRAM

The care management program provides services and supports that will assist people to age in place who might otherwise be in a long-term care facility.
Eligibility:
Any person aged 60 and over who is medically complex, at risk of long term care facility placement or currently resides in long-term care and would like to return home but may need services to reach this goal.
Care Management Team:
A registered nurse and social worker visit the client's home to evaluate health and social functioning, informal supports, options for other helpful programs, and the physical environment. The team works with the client and significant others to develop a plan that supports the client's wishes.
Services Available:
Services are provided on a cost-sharing basis, which is determined by a sliding fee scale based on income and expenses.
Local providers are contracted to provide the following services: Personal Care, Homemaking, Medication Management, Personal Emergency Response System, In-Home Respite Care, Home Delivered Meals and Nutritional Supplements, Adult Day Services and Transportation.
