MI Choice Waiver
The MI Choice Waiver program provides in-home services for low-income, Medicaid-eligible adults of all ages who are medically frail and whose needs would otherwise require nursing home care. Nurse and social worker teams work with participants to develop a person-centered, individualized plan that includes a mix of services appropriate to their needs and preferences. Waiver services are provided at the participant's home, whether living alone or with family, and may be available to residents of licensed adult foster care and homes for the aged when program guidelines are met.
Kalamazoo County has two waiver agents. You may choose to contact one or both.
CentraCare is part of the National Pace (Program of All-Inclusive Care for the Elderly) Association. It is a medical/social-based model of care that provides comprehensive integrated managed care to older adults who are at risk for nursing home placement. CentraCare provides services for residents of Kalamazoo County who are age 55 & up. In most cases, Medicare and Medicaid cover all services provided. Participants are transported to the Day Health Center located near downtown Kalamazoo, where a variety of services are offered such as recreational activities, a hot lunch, and therapy and medical appointments. Services may also be offered at the participant's home.
