We cannot replace our lost or misplaced COVID-19 Vaccine Card, but we can provide you with an official immunization record that documents that you received the COVID-19 vaccine. You can request an immunization record from the health department at 269-373-5200 or 269-373-5237. It may take up to two weeks to receive your record.
Michigan residents age 18 and older can also go to the Michigan Immunization Portal to access their vaccination records. To ensure privacy, individuals will be asked to create a MILogin account and upload a valid government-issued photo ID. There is no cost to access this portal.
A new nationwide Test to Treat initiative provides quick access to free treatment for COVID-19. Through this program, people are able to get tested and – if they are positive and treatments are appropriate for them – receive a prescription from a health care provider, and have their prescription filled all at one location.
These new sites are available at hundreds of locations nationwide, a Test to Treat locator is available to help find participating sites throughout the U.S. A call center is also available at 1-800-232-0233 (TTY 1-888-720-7489) from 8:00 am to midnight ET, 7 days a week. In addition, a Disability Information and Access Line (DIAL) is available to help people with disabilities access services. Free, rapid, at-home test kits are also available to residents of eligible communities in Michigan. Visit https://www.accesscovidtests.org/ to see if tests are available in your area.
The Michigan Department of Health and Human Services (MDHHS) has also provided free, at-home testing kits to selected libraries throughout the state. In Kalamazoo County, test kits are available, during business hours, at the Kalamazoo Public Library and the Portage District Library.
