- State I.D.
- Work or School I.D.
- Passport or Green Card
- Dept. of Corrections Printout
For children under the age of 18 years, the parent, legal guardian, or other authorized adult must provide proper picture I.D.

The Kalamazoo County Oral Health Program's mission is to provide access to preventive and restorative dental care to underprivileged and underserved residents of Kalamazoo County. Our vision for this Oral Health Program is to prevent unnecessary dental decay and to build lifelong dental health habits among the people we serve.
