The Kalamazoo County Oral Health Program's mission is to provide access to preventive and restorative dental care to underprivileged and underserved residents of Kalamazoo County. Our vision for this Oral Health Program is to prevent unnecessary dental decay and to build lifelong dental health habits among the people we serve.
What kind of services are provided at the Dental Clinic?
Effective January 1, 2016…Payment for services through the County Dental Program will operate under a sliding fee scale.
Payment and fees will be adjusted for individuals and families at or below 200% Federal Poverty Level.
Sliding Scale Fee Application and Income Verification Requirements
We're helping protect your identity. Patients will no longer be seen without proper picture identification:
- Drivers License
- State I.D.
- Work or School I.D.
- Passport or Green Card
- Dept. of Corrections Printout
For children under the age of 18 years, the parent, legal guardian, or other authorized adult must provide proper picture I.D.
SOME FORM OF PICTURE I.D. WILL BE REQUIRED!
