Case Management
Because they are experts in maternal and infant
health, Case Managers can answer your questions about pregnancy. In order to qualify for Case
Management you must be in your first or second trimester and living in one of the target zip
codes (see Who's Eligible).
Case Managers provide one-on-one support throughout your pregnancy.
During a visit with a Case Manager you may discuss:

 | Domestic violence |
 | Nutrition |
 | Mental health issues |

Need Help Accessing Services?
Case Managers have many connections in the community so they can help you access
resources you may not know about. They can connect you with things like:

 | Prenatal care |
 | Family planning (birth control) |
 | Parenting classes |

Goals for Case Managed participants
Among case managed program participants, the goals are to:
 | Reduce the number of low birth weight babies (babies weighing less than 5 lbs) |
 | Increase the number of women accessing first trimester prenatal care |
 | Ensure an adequate weight gain among pregnant participants (at least 16 lbs) |
 | Reduce the incidence of premature labor |
 | Promote healthy child spacing (at least 18 months between pregnancies) |
