Infant, children, and adult immunizations are available by appointment. IMPORTANT: bring a picture I.D., immunization record(s), and all insurance cards.

Off-Site Clinics: (No Appointment Necessary)

  • Portage Community Center, 325 East Centre Street, 2:30p.m.-4:30p.m., 1st Tuesday of the month


We're helping protect your identity. Patients will no longer be seen without proper picture identification:

  • Drivers License
  • State I.D.
  • Work or School I.D.
  • Passport or Green Card
  • Dept. of Corrections Printout

For children under the age of 18 years, the parent, legal guardian, or other authorized adult must provide proper picture I.D.

SOME FORM OF PICTURE I.D. WILL BE REQUIRED!
