Personal Action Toward Health (PATH): Developed and tested by Stanford University, PATH helps people learn tools and skills for the day-to-day management of long-term health conditions like arthritis, asthma, diabetes, depression, fibromyalgia and heart disease. PATH helps each person better manage their health conditions and their caregivers; Diabetes PATH and Chronic Pain PATH focus on specific conditions. The 6-week workshop meets for 2 hours each week; it is led by trained lay leaders.
PATH classes are offered in six (6) week sessions; each class is 2.5 hours in length. You will meet once a week over the six (6) week period.
