- Integrated Services of Kalamazoo: ISK is designed to provide assistance for individuals with a mental health or substance abuse problem. - BridgeWays Case Management: BridgeWays is designed to provide the highest quality case management and peer support services for individuals who experience symptoms of mental illness and co-occurring disorders. Michigan Mental Health Rules:
- The Michigan Rule of Court are the rules adopted by the Michigan Supreme Court to govern Michigan's legal system. The purpose of the Court Rules is to establish uniform rules and procedures for all levels of Michigan's court system. Please click on the title above to access this resource (please note: the Mental Health Rules start at Subchapter 5.730).
Court Forms:
- All SCAO – Approved court forms related to Mental Health Proceedings can be located at Court Forms
- If you are an attorney, agency, hospital, case worker or party who would like to receive correspondence by email, please download the Email Agreement Form, completely fill out the form and return to Kalamazoo County Probate Court. You can email the form to us at, KalProbateMentals@kalcounty.com.
- If you are an attorney who was appointed to represent a Kalamazoo County resident in a Mental Health case and are wishing to be paid for your services, please download the Court Appointed Representative Voucher for Services form, completely fill out the form and return to Kalamazoo County Probate Court. You can email the form to us at, KalProabteMentals@kalcounty.com.
- Guide on how to complete the Court Appointed Representative Voucher for Services form:
- You must list a File Number.
- You must list your client's name.
- For Mental Health Services:
- You must choose which county's rate you wish to be paid at. NOTE: if you select that you wish to be paid at Kalamazoo County's rate, you must select that option for any and all future Mental Health cases – you cannot select Kalamazoo County for one case then select to be paid at Calhoun County's rate for another case. If you select that you wish to be paid at another county's rate, you must use that county's rate of pay for any and all future Mental Health cases.
- For Kalamazoo County: you will be paid for each Event you do i.e. if you have a Deferral Hearing AND a 60 Day Hearing (Initial Petition Hearing), you will enter the dates and the amounts in the proper boxes. You will then list the total amount ($50.00 plus $125.00) in the TOTAL PAYMENT DUE box
- For other counties: please follow their guidelines and court rules. You must attach a billing statement.
*All out of county rates will be verified*
- You must enter an amount in the Total Payment Due box.
- Please date, sign your name, print your name legibly, enter your address and phone number.
- Do not leave any corresponding box blank – if you do not know any of the above information, please call (269) 383-8665 and we will assist you to the best of our ability.
- If you are an attorney who has not represented a Kalamazoo County resident before, you must submit a W-9 with your form, this is so we can get you set up with a Vendor Number and can process payments for you.
- Failure to follow this guide could result in a delay of payment.
