The Choices for Independence Program

The Choices for Independence (CFI) Program provides community-based case management for Kalamazoo County residents aged 60 and over. A team of Licensed Social Workers and Registered Nurses (Care Consultants) help promote dignity and independence through person-centered planning and coordination of supportive services. These services may include coordination of family supports, navigation of community resources, referring to community programs, counseling, caregiver respite, homemaking services, or personal care assistance.
