Information & Assistance

Free, unbiased assistance in accessing resources for senior citizens and their caregivers. Clinical social workers answer the call to determining needs, finding resources, facilitating referrals.

  • Unbiased evaluation of need and realistic options for support
  • Assistance navigating person centered care
  • Reliable, updated community resources

Assists with accessing resources to help with:

  • Basic needs:
    • Food (home delivered meals or meal sites)
    • Housing
    • Transportation
    • Assistance in the home with activities of daily living
  • Complex Needs:
    • Understanding Insurance
    • Accessing legal assistance
    • Advocacy
    • Health & Wellness programming
  • Evaluation and referral for resources
    • Area Agency on Aging Programs
    • Local, Regional, National Programs
  • Caregiver Resources
    • Understanding Dementia
    • Respite Resources

What to expect when you call:

  • The social worker will ask a few questions to assist in accessing appropriate resources.
