Schedule Your Vaccine Appointment

If you are over the age of 12 and live in Kalamazoo County, then you're eligible for a vaccine. See information on signing up here. You may also call the health department’s COVID-19 Call Center at 269-373-5200 to schedule your appointment.

Missed or Cancelled 2nd Dose Appointments

Those who have missed, cancelled or need to reschedule their 2nd dose appointment may do so by selecting a new appointment time here. Your 2nd dose must be the same product as your 1st dose. Do not schedule your 2nd dose earlier than 21 days if you received Pfizer or 28 days if you received Moderna. If you cannot remember which vaccine product you received or the date of your 1st dose, then you can call the COVID-19 Call Center at 269-373-5200 for assistance.

COVID-19 Vaccination Card Replacement

If you have lost or misplaced you COVID-19 Vaccine Card then you can request a copy of your immunization records from your primary care provider (doctor’s office) or the health department. We cannot replace your COVID-19 Vaccine Card, but your official immunization records will document you received the vaccine. Request immunization records from the health department by completing this form.

Report Out of State or Veterans Affairs Office Vaccinations

If you are a Kalamazoo County resident and were vaccinated in another state or at the VA (Veterans Affairs) Office, and want to be included in the MCIR registry, please complete this online form. Health Department staff will contact you within two weeks to collect additional information. If you are unable to complete the online form, call 269-373-5200 and select the option for the COVID-19 Call Center for assistance. You will be asked to provide proof of your vaccination.
