Immunization Record Request

Anyone 18 years or older, who has immunization records in MCIR, can locate their immunization record in MCIR if a record exists and matches their government issued ID (State Driver’s License or ID card, or US Passport). The immunization record from MCIR will appear as a .pdf document which can be downloaded, saved, or printed. Go to www.michigan.gov/immunize or at Michigan.gov/MiImmsPortal.

By completing the form below you are requesting an official immunization record from the health department. You should anticipate a call within a week to confirm your information. Once the request is processed, you may receive your official immunization record by mail or by picking up at the health department’s office. Please note, as of August 31, 2021, due to high request volume it may take up to two weeks to receive a phone call from the health department.
