Functions as a team nurse and Care Consultant within the Choices for Independence team; responsible for completing and maintaining client assessments/reassessments, and developing and implementing care plans to provide long term care management services for program clients. Connects clients with supportive services in the community to maintain their independence and prevent or delay long term care placement.
Conduct comprehensive client assessments to determine psychosocial, financial, and environmental strengths and limitations as well as medical status/conditions of potential clients. Develop plan of care in conjunction with the client, designated caregivers and Choices for Independence Social Worker(s). Monitor client condition and provision of service pertaining to plans for care. Adjust plan as determined through reassessment, normal monitoring and client's desired outcomes. Work with family members and volunteer caregivers to maximize available informal supports. Provide feedback to AAA staff regarding any problems or concerns with service delivery from purchase of service providers. Establish and maintain liaison with hospital discharge planners, human service providers, home care agencies, long-term care facilities, Department of Human Services and other referring agencies.
Current Michigan Registered Nurse License with an emphasis on gerontology and three years’ experience working with older adults; or any comparable combination of education, training or experience. Extensive interviewing and assessment skills and highly developed organizational skills. Excellent verbal and written communication skills. Knowledge of community resources, especially geriatric and chronic disease management. Reliable transportation and current Michigan driver’s license.
