The principle function of the job is to provide care coordination/case management services in the home/office setting encompassing psychosocial, medical, educational, and functional assessment and intervention for children with disabling conditions. This is done in order to assure that all children with disabling conditions reach their full potential, through advocacy and education of the family, thus improving the overall health of the community.
Provide case management/care coordination for recipients who have complex medical care and/or complex psycho‐social issues. Provide appropriate resources for recipient and family. Provide monitoring and follow‐up per Plans of Care. Resolve billing issues with families and providers. Develop pool of providers that provide services for the recipient and family.
Bachelor of Science in Nursing (BSN) with minimum two years’ experience in Public Health Nursing, Maternal and Child Health or Pediatric Clinical Care is required, or a Diploma in Nursing with minimum of three years’ experience, or an Associate Degree in Nursing (ADN) with minimum of four years’ experience, or military equivalency. Current Michigan Nursing license is required.
